OK, ssh_config SAYS: "The host is the hostname argument given on the command line (i.e., the name is 
not converted to a canonicalized host name before matching)."

But, really, I'd like to see case insensitivity.

MYHOST.somedomain.com is certainly the same as myhost.somedomain.com

This on OSX 10.3

yes, we know. Unfortunately there are lots of working configs that would break
if we started changing little things like this, so i think it is best to leave
it alone for now.

Don't worry, if we ever get around to thowing away the current client
configuration format, this would be high on the list.

Change all RESOLVED bug to CLOSED with the exception of the ones fixed post-4.4.
